Insurance and Billing
We accept the following carriers:

We accept all major insurance carriers. As a courtesy to our patients, we are happy to assist in obtaining your insurance benefits. You are responsible for knowing your plan's deductibles, yearly maximums, limitations and waiting periods. Please note that we accept many different plans and they all have different types of coverage. We will do our best to help you understand your individual insurance plan. Contact our office to help verify your insurance coverage and benefits.


Payment Options
We appreciate the opportunity to provide your dental care. The ability to pay is a concern to many dental patients. We share your concern and are prepared to offer you a number of payment options.

  1. Patient without dental Insurance need to know ....
    The fee for services rendered must be paid in full on the day of service. We do offer a discount on all payments that do not involve insurance when paid on the day of service.
  2. Patients with insurance coverage need to know ....
    If you have dental insurance, we file claims as a courtesy for our patients. We accept most traditional insurance plans. Please contact our office to verify acceptance of your plan. For certain procedures, your portion is due at time of treatment.
  3. We accept Visa, Mastercard, checks and cash for payment of the amount due.
    Cash discount of 5% for procedures not covered by insurance when paid at time of service with payment of cash or check.
  4. Care Credit is available for those patients who do not have insurance or need financial assistance. We offer a 12-month no interest financing plan. Additional information is available at Carecredit.com.

Emergency and After Hours
We know dental emergencies are never convenient or timely and we are committed to our patients' health no matter what time. If you are a current patient, and have a dental emergency after regular office hours, please call us at 414-475-5505.

Cancellation Policy
If you are unable to keep an appointment, we ask that you kindly provide us with at least a 24 hour notice from the time of your scheduled appointment. We ask for this advance notice so that we can offer this appointment to another patient.

We understand that occasionally unforeseen situations may arise which may require you to reschedule your appointed time. A fee may be charged If a patient repeatedly does not show up for an appointment or cancels appointments without sufficient notice.
